Warner Soccer Training Center Residential Required Forms
Please find BELOW the required forms (PDF format). Please print, complete and send in to Warner Soccer/1695-4 Metropolitan Cir/Tallahassee, FL 32308.
Forms are Due one week prior to camp.
Also below is a check off list of thing to bring from sheets to shoes.
Check in Sunday between 12:00-1:00pm (Dinner is first meal served at camp)
Check out Time Thursday between 5:00-6:00pm (Lunch is last meal served at camp)
Each participant must have insurance to participate in the program.
Each participant must complete and NOTARIZED the Medical Release Form.
Each participant must complete the rules form and be signed by both participant and parent.
Each participant must photocopy front and back of the insurance car.
Each participant must have $20.00 Loss Key check made payable to Warner Soccer (will shred upon return of dorm key or returned by request)
Participants will not be allowed to check in without the required forms(see below).
